- BCB Diploma Request Form (to be used only by BCB graduates. *Please note that diplomas will only be sent to graduates who complete this form.)
- Request a Verification of Enrollment Letter (login required)
- Cross-Listed Course Registration Approval Form for Boston Conservatory at Berklee and Berklee College of Music (login required. This form is to request registration permission for courses at Berklee College of Music that are not listed in Self-Service. Your requests will be submitted to the appropriate Department Chair for review.)
- Family Educational Rights and Privacy Act (FERPA) Release
- Immunization Record Request form (login required)
- Incomplete Grade Application (Students should now speak with their instructor to request a grade of incomplete. If the instructor agrees to award an incomplete grade she/he will complete an electronic form, which will notify the Office of the Registrar to award the incomplete grade.)
- Independent Study Proposal Form (for students who wish to propose an Independent Study for the following semester)
