Faculty and staff who would like to report unsafe behavior (e.g., a community member not wearing a mask or abiding by other safety protocols) should complete this form.

Consequences for Community Members Who Do Not Follow Safety Protocols

Community members who do not follow health and safety protocols will be managed on a case-by-case basis in consultation with Human Resources and the Office of Community Standards and Conflict Resolution.

  • For students, noncompliance could also lead to disciplinary probation, suspension, loss of on-campus housing, or loss of rehearsal and practice space.
  • For faculty and staff, continued noncompliance may result in restricted access to campus or removal of one's ability to work on the physical campus.
