Berklee High School Jazz Festival Scholarship Nomination Form

All nominations are due by February 13, 2019.

The guidelines are as follows:

Nominees must participate in the 2019 Berklee High School Jazz Festival.
Nominees may be either instrumentalists or vocalists.
Nominees must be at least 15 years of age by the end of the summer program to which the award applies.
Nominees may receive a maximum of one award per event.
